if anyone figures out a way to cheat the system and promote to an adult account, please send me a voice note on LIVE
GT whereforelamour

PS I've already changed my @live account to 1990, but the "Promote to Adult Account" tab isn't under Account Management

I had the same problem as you, as I couldn't see the Promote Child account option either.

So here's what I did.

I logged into my email account associated with my Xbox Live account. I went to my account settings on my email and changed my date of birth to 1991 so instead of being 16, Im 18 now.

After that, I went onto the Xbox and went to the Dashboard, chose My Xbox, then clicked on my gamer profile, chose Manage Account, and promote Child Account.
But, When I tried to do that, I would get an error a second into updating it.

So then I went into System Settings, Memory, then Format memory so everything off the Console was gone.

After that I went and tried to create a New gamer Profile. After that I pressed Join XBOX Live and used the email I used before.

Before logging on to XBOX Live It updated itself, then restarted the Console. Then I went back and finished recovering my Gamer tag.

After that, the Promote Child Account option worked perfectly, Now I'm enjoying Twitter, Facebook, and Last.fm.

Good Luck!
References :
I have the exact same problem. I was young when I got xbox live on the first xbox and I didn't have the foresight to lie about my age. I'm the only one of my friends that has a child account (because they started with the 360) and I'm extremely pissed about not being able to use last.fm and facebook especially. These are sites that don't require you to be 18 anyway, so what gives?

I can't start a new account because I have over 90 friends and 44,000 achievement points. Does anyone know a solution?

I've had so many problems because of this damn child account, including my account being unusable for over a month. Seriously, so close to the age. Why can't I get promoted?
